Objectives: Gain understanding of risk adjustment methodology and its role in Value Based Care models. Review specific official ICD-10-CM coding guidelines and how they are applied to diagnosis coding and reporting. Identify elements of the medical record that help support the validation of reported conditions. Identify documentation deficiencies for specific conditions.
1. Review patient cases and all necessary findings (pathology, radiology, surgery, etc.) and discuss key findings to determine course of treatment. 2. Discuss implications amongst multidisciplinary team members, and review best practices, current evidence-based findings, etc. 3. Assign treatment protocols based on consensus of the multidisciplinary team.
