• At the conclusion, the participants should be able to DISCUSS a clinical case of a patient transition in care; • NAME two patient characteristics that help assess risk of sub-optimal transitions in care, Using excepts from required reading for our students, Kim CS, Flanders SA; “Transitions of Care” Annals of Internal Medicine 158 (5) ITC 2-16 • DESCRIBE the ‘teach back’ method to assess patient and /or family understanding a plan of care. • DISCUSS the case and NAME one intervention to improve the transition in care discussed. • DISCUSS a clinical example appropriate for fall risk assessment. • RECOGNIZE the RITUAL named in the CDC brochure “Talking With Your Patient about Falls” pocket guide with reference to an algorithm. • USE at least two elements of the Algorithm for Fall Risk Assessment Interventions • NAME and RECOGNIZE one test of gait, strength, and balance reference in the Fall Risk Checklist for clinicians. Questions and answers will allow for audience participation.
