Chronic Care Clerkship Faculty Meeting
At the conclusion, the participant should be able to:
1) DEMONSTRATE knowledge of the natural history of common chronic diseases and the interventions that result in favorable outcomes; 2) DEMONSTRATE knowledge of the factors that contribute to adherence or non-compliance with chronic disease care including psychological, social and behavioral; 3) APPLY the principles of prevention and chronic disease management to patients, families and populations; 4) DEMONSTRATE the ability to document the care of chronically ill patients by writing concise, clear and useful SOAP progress notes. Questions and answers will allow for audience participation.
