Making it Safe to Grow Old and Frail: The MediCaring Reform
At the conclusion, the participant should be able to 1) ENABLE identification and assessment of frail elders in routine settings; 2) ENGENDER commitment to good care plans and care plan processes; and 3) EXPLORE opportunities for ACO funding and local management. Questions and answers will allow for audience participation.
