Education Director's Meeting for Geriatrics: Geriatrics Risk Assessment

• At the conclusion, the participants should be able to:

1) DISCUSS a clinical case of a patient transition in care; 2) NAME two patient characteristics that help assess risk of sub-optimal transitions in care, using excerpts from required reading for our students, Kim CS, Flanders SA; “Transitions of Care” Annals of Internal Medicine 158 (5) ITC 2-16; 3) DESCRIBE the ‘teach back’ method to assess patient and/or family understanding of a plan of care; 4) NAME one intervention to improve the transition in care discussed. Fall Risk Assessment Competency Objectives: (from slide 10 in PowerPoint); 1) DISCUSS a clinical example appropriate for fall risk assessment; RECOGNIZE the RITUAL named in the CDC brochure “Talking With Your Patient about Falls” pocket guide with reference to an algorithm; 2) USE at least two elements of the algorithm for Fall Risk Assessment Interventions http://www.cdc.gov/homeandrecreationalsafety/pdf/steadi/pocket_guide_preventing-falls.pdf in the case discussed; 3) NAME and RECOGNIZE one test of gait, strength, and balance reference in the Fall Risk Checklist for clinicians at http://www.cdc.gov/HomeandRecreationalSafety/pdf/steadi/fall_risk_checklist.pdf.
