At the conclusion, the participant should be able to:
1) DEMONSTRATE knowledge of the natural history of a variety of common chronic diseases; 2) DEMONSTRATE knowledge of the factors that contribute to compliance with chronic disease care including psychological, social, and behavioral; 3) APPLY the principles of prevention and chronic disease management to patients, families, and populations in the Primary care setting; 4) DEMONSTRATE the ability to document and monitor disease progression and health maintenance/prevention in patients with chronic disease; 5) DEMONSTRATE continuity of care over the entire third year of medical school. Questions and answers will allow for audience participation.
