SUMMARY:Genetic Case Studies
DESCRIPTION:Genetic etiologies are the most common cause of pre-lingual hearing loss and deafness. Identification of a genetic diagnosis of hearing loss allows for tailored medical and audiological management as well as genetic counseling. In this session\, two genetic case studies will be presented by interdisciplinary teams including otolaryngologists\, mid-level providers\, and genetic counselors. Presenters will describe the characteristics of the genetic diagnoses as well as implications for treatment and management of these patients. \nThe audience will see two case studies:\n1. The first case is a six-year-old male who recently underwent cholesteatoma surgery. A complicated case in that this patient had a congenital cholesteatoma and stigmata of branchiotorenal syndrome. Of great interest\, the patient’s mother had the same findings. This is an extremely rare case and likely represents an inherited condition.\n2. The second case reviews the early genetic diagnosis of DFNX2 hearing loss which was critical in subsequent management and counseling of the patient and family. \n  \n\n\n\nThe attendees will be able to:\n1. Describe the characteristics of BOR syndrome.\n2. Develop a diagnosis and treatment plan for patients exhibiting signs and symptoms of BOR syndrome.\n3. Summarize key features of DFNX2 (POU3F4-related hearing loss) and impact on surgical management\, audiologic habilitation\, and family counseling.\n4. Illustrate interdisciplinary approach to caring for patient with genetic diagnosis of hearing loss.\n\n\n\n

SUMMARY:Otoacoustic Emissions and Hidden Hearing Loss
DESCRIPTION:While the audiogram is a widely used diagnostic tool\, it is a poor representation of a person’s underlying otopathology and can therefore be a poor predictor of a person’s hearing impairment. For example\, persons with audiometric thresholds within normal limits may experience hearing impairment such as difficulty hearing in noise. Cochlear synaptopathy and outer hair cell dysfunction are two otopathologies undetected by the standard audiogram (a.k.a. Hidden Hearing Loss)\, but outer hair cell function plays a primary role in hearing in noise performance. A third undetected otopathology is cochlear untuning\, which occurs secondary to outer hair cell damage. This talk will discuss each of these otopathologies and present clinical normative data that can be used to differentially diagnose each otopathology. \nThe attendees will be able to:\n1. SUMMARY:Helping Patients with Hearing Loss Overcome Barriers to Surgical Procedures
DESCRIPTION:Advances in medical/surgical intervention for hearing loss provide more alternatives than ever before for patients with progressive hearing loss. One of the most challenging situations\, however\, for both patients and providers is to consider WHEN traditional hearing aids are not enough. This panel session will consider the evidence basis\, clinical considerations\, and ethical dilemmas that often surround the decision to proceed with medical/surgical treatments for pediatric and adult otologic patients. Audience participation is strongly encouraged! \nAttendees will be able to:\n1. List three objections listed by patients for why they delay medical/surgical treatments for hearing loss\n2. Provide the evidence basis for cochlear implant candidacy\, in terms of hearing thresholds and speech recognition scores\n3. Discuss three clinical challenges with bilateral implants\, hybrid/bimodal stimulation\, middle-ear implantable devices\, and bone-anchored devices that may provide barriers to medical/surgical intervention.
