SUMMARY:Clinical Trials for Emerging Hearing Therapies
DESCRIPTION:Besides rehabilitation approaches with hearing aids\, there are no available treatment options for permanent hearing loss; however\, significant contributions to the state of the science has been made at the drug discovery level\, and with first-in-kind clinical trials\, along with significant resources being allocated in the pursuit of more options for those with hearing loss. In fact\, there are more than 40 pharmaceutical and biotechnology companies developing inner ear therapies and potential drug delivery systems. This session will present an overview of the hearing loss therapeutic landscape. We will review groundbreaking clinical trials completed\, in progress\, and on the horizon. The studies reviewed will include gene therapies along with preventative\, enhancement\, and regenerative medicine approaches. The review will empower clinicians to be aware of the most recent developments and be conscious of the evolving future landscape of treatment and management of hearing loss. Clinicians have an important role to play\, both in educating their patients about the current state of novel therapies\, and in partnering in the development and evaluation of these promising interventions. \nAfter this session\, attendees will be able to:\n1. Describe at least (3) novel treatment approaches underway to prevent\, enhancement\, or regenerate and treat hearing loss.\n2. Describe the drug discovery path from translation into practice\, including the need for animal model experiments\, FDA-regulated clinical trials\, and then on to marketing.\n3. Identify at least two (2) resources that can be referenced by clinicians or shared with patients on current clinical trials and possible novel therapies.

DESCRIPTION:As indications for cochlear implantation expand\, children with significant unilateral hearing loss (UHL) have become candidates. Children with UHL face unique difficulties with localization\, hearing in noise\, and speech recognition that trickle down into their performance in school and their ability to meet their full potential. This session will provide evidence-based guidance for cochlear implant candidacy\, management\, and expected outcomes in children with UHL. The speakers will focus on 24-month performance data obtained as part of a clinical trial investigating the safety and efficacy of cochlear implantation in this population. \nAs a result of this course\, participants will be able to:\n1) Describe characteristics of children with severe-to-profound unilateral sensorineural hearing loss.\n2) Discuss factors that may influence outcomes in children with cochlear implants and normal hearing in the contralateral ear.\n3) Identify potential benefits children with unilateral hearing loss may experience when using a cochlear implant.

SUMMARY:When to Refer for CI and What Happens Next
DESCRIPTION:Cochlear implants (CIs) have been FDA-approved for both children and adults for more than 30 years\, and even though they are widely accepted as the preferred hearing treatment for patients with significant hearing loss\, great confusion exists regarding when it is best to refer a patient for a CI candidacy evaluation. This is particularly true in recent years as candidacy has focused less on the patient’s best aided performance and has placed greater emphasis on the ear to be treated\, as evidenced by the FDA’s recent approval for CIs in cases of single sided deafness (SSD) and asymmetric hearing loss (AHL). During this presentation\, we will review basic audiometric test results that can be used to determine if a patient should be referred for a CI candidacy evaluation. The discussion will include a review of the 60/60 – a guideline developed to assist clinicians when making decisions regarding such a referral.\nOnce a patient is referred to a cochlear implant (CI) center\, the audiologic portion of the CI evaluation is the first step in determining candidacy. While hearing aid verification\, aided speech perception testing\, and device counseling are key components of this appointment\, it is also crucial to evaluate the whole patient and assess if they would truly benefit from CI technology. In this session\, we will review the importance of each step of the CI evaluation\, and how this appointment can impact patient expectations\, post operative outcomes and insurance decisions.\nThis session will also address the medical and surgical considerations in cochlear implantation. Hearing loss may progress beyond what hearing-aid amplification can adequately address. Patients with inadequately habilitated hearing loss are at risk of social isolation\, depression\, and earlier dementia\, necessitating appropriate intervention with cochlear implants. Several medical considerations exist\, including anatomic suitability\, medical fitness for surgery and device considerations. A great deal of disinformation must also be overcome as to the nature and risks of the surgery\, which in experienced hands is trivial. These considerations will be presented from an ear surgeon’s perspective. \nObjectives\n• At the end of this presentation\, participants will be able to describe the 60/60 Guideline for Referring Adults for a Cochlear Implant Candidacy Evaluation.\n• At the end of this presentation\, participants will be able to discuss the medical implications of untreated or under-treated hearing loss.\n• At the end of this presentation\, participants will be able to describe two key components of the cochlear implant evaluation.

SUMMARY:Case Presentations with Single-Sided Deafness
DESCRIPTION:“This session will include a discussion around the presentation of 4 case presentations. The first presentation will highlight the importance of full-time device use in cases of pediatric SSD. Two cases will be contrasted\, both with the same etiology and similar thresholds at presentation. One patient wore her device full-time while the other had extremely limited use. Both children had a change in hearing in their better hearing ear which impacted their overall abilities socially and academically. Outcomes between cases are significantly different\, particularly since they now have bilateral hearing loss. The full-time device user has maintained bilateral hearing while the other remains a monaural listener. \nThe second case will address the importance of frequency-place stimulation in the cochlea in cases of cochlear implantation for unilateral hearing loss. Unilateral sensory hearing loss has been approved as an indication for cochlear implantation for two of the cochlear implant manufacturers. Providing a cochlear implant signal that faithfully replicates what is heard acoustically contralaterally is of paramount importance to successful outcomes in cochlear implantation for unilateral hearing loss. We have determined that aligning the frequency signal from the cochlear implant to natural cochlear frequency place enhances patient outcomes\, especially in those situations that require binaural cues such as speech perception in noise. \nAnd much more.” \nObjectives\n• Participants will be able to describe the importance of aligning frequency and place with a cochlear implant placed for unilateral hearing loss.\n• Participants will be able to recall a risk factor for progression to bilateral hearing loss from SSD.

DESCRIPTION:Traumatic brain injuries and concussions cause complex physical\, emotional\, and cognitive symptoms. More specifically\, the disruption of sensory input can affect the integration of input in the brain leading to disordered motor output\, thus involving the ear and balance system. It is imperative to provide these patients with a careful\, comprehensive\, and integrative approach to differential diagnosis and management starting with the ear. SUMMARY:Genetic Case Studies
DESCRIPTION:Genetic etiologies are the most common cause of pre-lingual hearing loss and deafness. Identification of a genetic diagnosis of hearing loss allows for tailored medical and audiological management as well as genetic counseling. In this session\, two genetic case studies will be presented by interdisciplinary teams including otolaryngologists\, mid-level providers\, and genetic counselors. Presenters will describe the characteristics of the genetic diagnoses as well as implications for treatment and management of these patients. \nThe audience will see two case studies:\n1. The first case is a six-year-old male who recently underwent cholesteatoma surgery. A complicated case in that this patient had a congenital cholesteatoma and stigmata of branchiotorenal syndrome. Of great interest\, the patient’s mother had the same findings. This is an extremely rare case and likely represents an inherited condition.\n2. The second case reviews the early genetic diagnosis of DFNX2 hearing loss which was critical in subsequent management and counseling of the patient and family. \n  \n\n\n\nThe attendees will be able to:\n1. Describe the characteristics of BOR syndrome.\n2. Develop a diagnosis and treatment plan for patients exhibiting signs and symptoms of BOR syndrome.\n3. Summarize key features of DFNX2 (POU3F4-related hearing loss) and impact on surgical management\, audiologic habilitation\, and family counseling.\n4. Illustrate interdisciplinary approach to caring for patient with genetic diagnosis of hearing loss.\n\n\n\n
