DESCRIPTION:Tinnitus affects more than 1 billion people on this planet. The primary association is sensorineural hearing loss from noise\, aging medications and more.  Tinnitus is a symptom and while there are no known cures there are many options to help our patients.  This session will update the attendee on Tinnitus and its management.  \nDescribe the underlying mechanisms and common causes of tinnitus associated with sensorineural hearing loss.\nDiscuss current evidence-based approaches and emerging therapies for the management of tinnitus.\nEvaluate recent advances in gene therapy research and their potential implications for future tinnitus treatment.

SUMMARY:FCOM 2025: Endoscopic Ear Surgery
DESCRIPTION:Endoscopic ear surgery (EES) has matured from a niche approach to a core technique at some centers to manage routine and complex tympanic membrane perforations\, cholesteatoma\, ossicular chain pathology\, and select lateral skull base conditions. The following presentation will synthesize current evidence and technical refinements that improve visualization\, reduce morbidity\, and expand candidacy for minimally invasive transcanal\, transmastoid\, and transcranial procedures. The rationale for endoscopic “heads up” otologic surgery will be discussed with a review of case-based pearls for challenging anatomy (e.g.\, sinus tympani\, anterior epitympanum)\, outcomes\, and complication. Participants will leave with practical algorithms for patient selection\, structured approaches to cholesteatoma clearance\, and strategies that enhance safety and patient-reported outcomes. Device and instrument options will be discussed comparatively using only published data. \nBy the end of this activity\, participants will be able to: \nDifferentiate indications and contraindications for endoscopic vs. microscopic ear surgery across common pathologies (perforation\, cholesteatoma\, ossicular chain reconstruction\, lateral skull base tumors\, superior canal dehiscence). \nApply step-wise techniques for safe transcanal endoscopic access\, hemostasis\, and instrument triaging in one-handed surgery. \nIdentify anatomic “blind spots” (e.g.\, sinus tympani\, facial recess\, anterior epitympanum) and implement visualization strategies to minimize residual disease. \nCompare short- and long-term outcomes (hearing results\, residual/recurrence\, complications) for EES relative to conventional approaches and integrate these data into procedure selection. \nConstruct a training and adoption plan—simulation\, video review\, instrumentation sets\, and OR workflow—that reduces the learning curve and improves team performance.

DESCRIPTION:This panel discussion will focus on questions asked at otolaryngic allergy meetings regarding patient selection for immunotherapy\, contraindications to testing or treatment\, allergen selection\, duration of each phase of immunotherapy\, and the location of allergy immunotherapy treatment. Experts in the field will discuss their practice experience and what the literature has to say about these issues. \nParticipants would be able to do the following: \n1. Understand the rationale for deciding whether or not to test and treat patients taking beta blockers. If testing\, should the testing be limited?\n2. Determine if epinephrine autoinjectors should be required for patients on injection immunotherapy.\n3. Decide if home or other out-of-office site of injection immunotherapy is appropriate.\n4. Understand how to select allergens for testing and treatment in their geographic area.\n5. Create treatment plans for patients who may reside out of state for part of the year.
