For Physicians:                    $120 early bird rate postmarked before 8/1/08

                                            $170 postmarked on or after 8/1/08

 

For Residents* and Fellows, Nurses, Mid-level practitioners & other health professionals:

                                           $ 65 early bird rate postmarked before 8/1/08

                                            $115 postmarked on or after 8/1/08

 

Sacred Heart Employees:  $ 15 early bird rate before 8/1/08 (Register through MOE)

                                           $115 postmarked after 8/1/08 (no discount after 8/1/08)

 

*With the exception of residents at Sacred Heart Women's and Children's Hospital, all other residents must send a letter from their director.
